Staff, residents, patients, community groups and local charities will all play their part in helping shape where and how care is delivered. As well as working with people to plan their care and prevent ill health we need staff to be part of our journey to creating a truly joined up care system.

The integrated care partnership will affect different people’s jobs differently, and for many staff there won’t be any change. However, for people who work in the community you may find you need to work slightly differently. One of the main changes will be that you will benefit from improved collaboration and closer working relationships with colleagues from different teams and different organisations. This will improve information sharing and decision and mean we can provide better care for local residents.
